Many people with shoulder pain have had this experience.

Stretching can actually help reduce tension in the muscles around the shoulder and temporarily make movement more comfortable. It can relax muscles that have shortened from holding the same posture for a long time and ease tension in surrounding tissues, reducing that stiff feeling.

However, if pain keeps returning to the same area even after stretching, it may be hard to explain it simply as tight muscles.

When there's a problem with the tendons or the tissues around the joint, separate from muscle tension, or when the way the shoulder moves has changed, stretching alone can't resolve the cause. In other words, feeling better after stretching and actually addressing the cause of pain can be two different things.

Shoulder Pain May Not Be Just a Muscle Problem

The shoulder isn't a joint moved by a single muscle.

Raising and rotating your arm involves several tendons known as the rotator cuff, the shoulder blade, and the tissues around the joint all working together. The rotator cuff in particular plays an important role in stabilizing the shoulder joint and controlling arm movement.

But with repetitive use or faulty movement, strain can keep building in this area. And if your shoulders round forward or your shoulder blade movement has changed, certain tissues can be repeatedly compressed when you move your arm.

In these situations, stretching that simply lengthens the surrounding muscles may not bring enough change. For example, if strain has built up in the tendons around the rotator cuff, the same area can be irritated again even after the muscles are loosened. And if joint movement is restricted, an approach that restores the movement itself may be needed instead.

It's Important to Identify the Problem

Not all shoulder pain comes from the same cause. Even when the same area hurts, one person's pain may come from simple muscle tension, while another's may come from changes in the tendons or tissues around the joint.

That's why diagnosis isn't just about "where it hurts." We also check:

  • When the pain started
  • Whether it gets worse with certain movements
  • How far you can move your arm
  • Whether you feel any loss of strength
  • Whether the pain gets worse at night

When needed, we use X-rays to check joint structure and alignment, and ultrasound to examine the tendons, including the rotator cuff, and the surrounding tissues.

This process isn't simply about locating the painful spot. It's about identifying which tissues are under strain and deciding on the right treatment approach.

The Treatment Approach Depends on the Cause

Not all shoulder pain can be managed with stretching or exercise alone.

If the tissues around tendons or ligaments have weakened, or healing has slowed due to repeated strain, treatments such as prolotherapy or collagen injections may be considered to support tissue recovery.

Prolotherapy involves injecting a treatment solution into weakened ligaments and tendons to stimulate the body's healing response, while collagen injections are used to help support an environment in which damaged tissue can recover.

When there is chronic tendon strain or delayed tissue healing, extracorporeal shockwave therapy (ESWT) may also be used to support the recovery process.

On the other hand, if shoulder movement is restricted or muscle function has declined, exercise therapy tailored to your current condition may be needed to restore movement.

In the end, what matters isn't stretching more, but choosing a management approach that fits the current state of your shoulder.

Your Shoulder's Current Condition Matters More Than Stretching

Stretching is one good way to care for your shoulders, but not all shoulder pain comes from simple muscle tension.

There can be many different causes, including rotator cuff tendon problems, calcific changes, restricted joint movement, and repeated strain on surrounding tissues, so the approach may differ depending on your symptoms.

In particular, if the same pain keeps coming back, your arm's range of motion has decreased, or you're having trouble with daily activities, it may be worth checking your current condition rather than just putting up with it.

What matters is not loosening things up no matter what, but understanding why the discomfort developed and managing it in the right way.
